Goodwill of the Heartland offers three options for individuals who experience barriers to independence:

Day Habilitation
Goodwill of the Heartland Day Habilitation services provide opportunities for participants to increase their independence through integration in the community, development and maintenance of independent living skills, selection of social and leisure activities, and maintenance of their physical and mental health. We promote self-advocacy and offer a program focused on personalized attention with an emphasis on individual abilities and needs.

Supported Community Living
Goodwill of the Heartland provides services to individuals to help them live independently within their own communities. We can assist with all aspects of skill development, including meal planning and preparation, household chore planning and completion, emergency situations, planning and scheduling, transportation, shopping, recreation and socialization activities, advocacy and services coordination, budgeting and assistance with paying bills. For those with chronic mental illness, Supported Community Living can focus on work and other activities that promote adjustment to disability and symptom management.
To enter the Supported Community Living service, individuals must:
- Be 18 years of age or older
- Have a disability or disadvantaging condition
- Be able to function in a community setting without 24-hour supervision

Case Management
Goodwill’s Case Management services are designed to assist individuals with brain injury with a successful re-entry into the community after injury. This service is offered for residents of Benton, Cedar, Linn, Iowa, Jones or Washington Counties.
The purpose of Case Management is to promote the greatest level of independence for individuals with brain injury and to empower individuals and their families to plan for future needs.
Case Management provides assessment and referral services for
individuals and their families. Current needs, as well as personal
interests and goals are identified. An individual’s needs
are assessed in a variety of life domains, including financial,
medical, legal, residential, vocational/academic and social/personal.
Once needs are identified and prioritized, individuals are linked
with the appropriate services. Services provided by multiple agencies
are monitored and coordinated. Each case is monitored to assess
satisfaction and progress toward goals and to address additional
needs or goals as needed.
Case Management also provides advocacy and community education
for individuals with brain injuries and their families.
Case Management Eligibility Requirements
- No age limitations
- Documentation of a brain injury or have a family member with
a brain injury
- Resident of Benton, Cedar, Linn, Iowa, Jones or Washington Counties
- Completion of the acute care phase of recovery
- Demonstrates willingness to participate actively in working
towards achievement of stated goals
